TPMS New Student Enrollment for 2011-12

In mid May, 2011,we will start collecting new student enrollment paperwork (students NEW to the Poway Unified School District) for the 2011-12 school year.

Once you have filled out the required Poway Unified School District forms (below), please return to TPMS website for our paperwork.

Required TPMS site specific forms to complete:

Appropriate Grade level Course Request Form

*** Helpful but not required: most recent progress report or report card and any Standardized test records or GATE certification (if applicable).

After you have completed the paperwork, bring it to Twin Peaks Administration/Counseling Office between the hours of 1:30 and 3:15 pm.  The enrollment process takes approximately 20 minutes. 

You must bring ALL of the following to enroll your student:

  • Enrollment Form                                   2 Proof of Residency documents (utility bills,
  • Signature Verification Form                    lease agreement, etc.)
  • Home Language Survey                          ORIGINAL Birth Certificate or Passport
  • Residency Verification Form                             Student’s Immunization Record***
  • Student Health Info Form                        ***Tdap Vaccine Verification 
  • Transfer of Student Records Request         7th and 8th grade students will NOT
  • TPMS Special Services Checklist               be allowed enroll without Tdap Verification
  • Course Request Form

Please call TPMS Counseling Office at 858-748-5131 x2815 to schedule a New Student appointment time if unable to visit between 1:30 and 3:15.

IMPORTANT: If you do not reside in Twin Peaks Middle School's boundaries, you will also need to apply for an Intradistrict transfer (within PUSD) district form PP-126 or an Interdistrict transfer (outside PUSD) SDCOE form 341. These forms are submitted to the PUSD Office located at 15250 Avenue of Science, San Diego, CA 92128-3406, not the school site. See 'Student Attendance and Discipline' for the proper form

Important note: We cannot enroll your student at our school until your application (Intradistrict or Interdistrict transfer) has been approved.

Special Circumstances:

  • If your student has been in Special Education (in resource or speech), please bring a copy of the latest Individual Education Plan (IEP) with you to your enrollment appointment.
  • If you are NOT the parent or court-appointed guardian,  additional documentation/forms are available at the school.
  • If your student is a Special Day Class student, please contact our District Special Education office at 858-668-4100.

Once your student is enrolled, you will be given a student ID number for your student. Your next step is to set up a parent account to allow access to www.learningpoint.org

Your parent login will be the student ID number with the letter ‘P’ in front - P{student ID} (ex. P1234567).  You must contact the learningpoint help desk to receive a password.
