GENERAL INFORMATION
Students must obtain a Health Office Pass or a note from
their teacher to go to the Health Office. All students must
sign in on the log sheet. Students who need to go to the
Health Office during lunch need permission from a Noon Duty of
office personnel. In case of an emergency, a pass permission
is not required.
Illness:
* With a temperature of 101 (or 100 with other symptoms),
student needs to go home
*With a normal temperature, students rest for 10-15 minutes and are
re-evaluated. At that point they either return to class OR
call a parent to discuss their situation.
Injury:
*Minor wounds are cleansed and bandaged, and the student is
sent back to class.
*Ice pack are given for sprains/strains.
*Serious injuries-parents are always called.
MEDICATION
PROCEDURES: (top)
According to California state
law, prescription and non-prescription medications are permitted to
be taken at school ONLY with a written statement from the physician
AND a written statement from the parent or guardian. The
Health Office has a form available entitled, "Parent Authorization
for Medication Administration" (PUSD Form H-26). Written
information that must be provided is the student's name, name of
medication, and physician's instructions detailing the method,
amount, and time the medication is to be given, as well as
parent/guardian and physician signature. This information is
required for all medication, including over-the-counter Tylenol,
Ibuprofen, cold/allergy medications, etc. All medication MUST
be in a bottle that is labeled with the student's name and the above
information. NO PLASTIC BAGGIES WILL BE ACCEPTED.
Students who must carry their medication with them (i.e. inhalers)
also need specific authorization to so do (use PUSD Form H-26B)
which is also available in the Health Office. Please call if
you have any questions or concerns.

Vision Screening:
6th grade screening will take place in CORE
classes

Please remind your student to bring his/her glasses to school on
the screening dates.
Vision and Hearing Screening:
8th grade screening will take place during CORE
classes

the screening dates.
Scoliosis Screening:
Grade 7 - Girls
Grade 8 - Boys
Scoliosis screening consists of a visual examination of the
student's back by a credentialed school nurse. Girls are asked to
wear a sports bra or bathing suit under their school clothes as
shirts/tops must be removed for proper screening. Appropriate
privacy is provided.
MEDICAL P.E. EXEMPTION

If a student is unable to participate in Physical Education
due to illness or injury, they must have a note from a
parent/guardian that states the following:
* Name of student
* Date
* Reason for exemption
* Parent/guardian name and signature
* Daytime phone number
A student may be excused for up to three (3) days with a parent
note. As a courtesy, if a parent phones the Health Technician
and makes a verbal request for P.E. exemption, a one day only
exemption will be written.
A physician must certify exemptions that exceed 3 days. This
should include the diagnosis/reason for exemption.
Students excused from participation are not necessarily excused from
dressing out or class attendance. If there is a severe
disability/illness, dressing out will not be required and student
will check into the library. They must have a signed pas from
a staff member to check into the library, and will sign into an
attendance book.
Long-term exemptions are handled on a case-by-case basis, but
must have physician documentation.
