SITE SPECIFIC FORMS FOR REGISTRATION:

 

Listed below are the site-specific forms necessary to complete your student’s registration for the 2010-2011 school year.  Please pay close attention to the ACTION REQUIRED column as this will provide you the directions for completing this process successfully.

BHMS Before and After School Program

BHMS provides free Before and After-School programming as part of California’s After-School Education and Safety program.  Before-School Program hours are 6:45 – 8:15 AM, Monday thru Thursday; and 6:45 – 9:15 AM on Fridays.  We offer a variety of after school clubs, sports, tutorials, homework assistance, arts and crafts and computers are available during our after school program..  Hours of operation are 3:15 – 6:15 PM.  See BHMS website or contact Debby Sather (485-4894; dsather@powayusd.com) for more information.

 

PTSA Membership

Complete and Return if desired on your assigned Registration Day.  Please help us out.  The BHMS PTSA (Parent Teacher Student Association) provides and supports activities and programs that enrich the educational experiences of every student at Bernardo Heights Middle School.  These programs and activities cannot take place without the funds raised through PTSA memberships such as yours.

Grocery Card Program

The Bernardo Heights Education Foundation is a volunteer organization dedicated to raising funds to purchase/upgrade classroom technology and to fund ongoing campus improvements.  All students benefit through these purchases. Last year's purchases included classroom book sets, classroom computers/licenses, classroom printers, and classroom LCD projectors.  The Foundation is an IRS recognized 501c3 organization, and any donations made to the Foundation are tax deductible.

Volunteer site transfer request

Print out, complete, and bring to registration to be part of the BHMS volunteer team.  A current TB test or chest X-ray report must be submitted with the volunteer paperwork

A Level I (Megan's Law) clearance is required to work under the supervision of a district employee.
 A Level II clearance is required to work independently without the supervision of a district employee.  For Level II paperwork please see Karen Rockafellor in the school office.

Required for all volunteers.

Complete only if you are already cleared as a Volunteer at another school in PUSD.

After School Enrichment Program  Registration Form

 

Be sure to check this out!!! The After-School Enrichment Program, run by KidzTyme, is designed to enhance and enrich students' lives, expand and extend educational opportunities, and constructively channel students' energies and provide meaningful activities for their free time.  Complete and Return if desired on your assigned Registration Day

REQUIRED—  Pictures will be taken at Registration and Instant ID cards will be issued.  As time permits, forms will be mailed to student homes in early August.  If you do not receive a form in the mail, please use the ones at Registration.
