Continuing Student Registration
Continuing student registration for the 2012-2013 school year will occur
on
Wednesday, May 23rd OR Thursday, May 24th from 2:00 - 4:00 pm.
This is required for all current Creekside students who will continue at this school
next year. Forms available at

- Student Information Form (available at the school at the time of registration)will be reviewed by parent for each student.
- Signature Verification page (available at the website above ) Signed by parent and student (if student is 4th or 5th grade).
- Student Health Information 4th grade only or if changes in health (available at the website above)
- CA Healthy Kids 5th grade only (available at the website above)
- Report of Physical Exam (CHDP) Required by October 1st of First Grade (available at the website above)
Class Placement Request
Classroom placement of students for the 2012-2013 school year involves teachers and administration deciding what we believe will be best for all students. If you have information regarding your child's needs (flexible/ structured environment, interests, talents, etc.) please use the form found here to put those thoughts in written form. Forms must be returned to the school office by June 1st.
New Student Enrollment
Enrollment for new students for the 2012-2013 school year will continue weekdays between 10:00 am and 2:00 pm until the end of school, June 7th. Enrollment forms and information are available at www.powayusd.com.

(your child must be 5 years old on or before Dec 2, 2012) -
Child's Immunization Record
5 doses DPT (4 doses if last dose was given after 4th birthday).
4 doses Polio (3 doses if last dose was given after 4th birthday).
3 doses Hepatitis B.
2 doses MMR. Varicella (chicken pox) immunization or physician’s documentation that child has had the disease -
2 documents showing proof of residence
(utility bills, bank statement, lease or rental agreement, escrow paper)
Volunteers
At Creekside we love and appreciate our volunteers. Please review the information below and bring these 3 required forms to the school office to be cleared to volunteer. It may take a few weeks to complete this process, so plan ahead!
TB Testing: Each volunteer must have a TB test done within the past 4 years on file with the school office. PUSD offers TB clinics to our volunteers. Volunteers need proof from their school they are indeed volunteers. Take this proof with you to the clinic. Click HERE for the schedule. You may also visit your doctor's office or a local clinic if our times do not fit your schedule, however there may be a cost for the TB test at these locations.
Megan’s Law Background Check: Each volunteer must have a current Megan’s Law Background Check on file with the school office. You do not need to fill out a new form for each school year. Click here to print the Megan's Law form then bring it to the school office with your identification and we'll get it started.
Volunteer Code of Conduct: Volunteers must have a Volunteer Code of Conduct form on file at each school where they volunteer. Click here to print this form.
Field Trip Drivers
At times, parents may be asked to drive students for a field trip. Before agreeing to drive, please provide the school office with the following information:
Private Vehicle Transportation Form: Click here to print this form. Please read this form thoroughly as it specifies how much insurance is required in order to drive students. Complete this and return to the school office along with a copy of your driver's license and a copy of the declarations page of your insurance policy.
Before School Supervision
The playground will be open to students in the morning beginning at 8:40 A.M. No supervision will be provided before 8:40 A.M. so students should not arrive at school before this time. Once students arrive, they may not leave unless a parent or other authorized person comes to the office to sign them out. To ensure the safety of our children, you will be required to show identification when you pick up your child.
Absences
The absence reporting phone number is 858-679-9343 and is available 24 hours a day. State law requires that attendance be verified. Please call the absence line each day your child is absent from school. In addition, if they will be tardy more than 30 minutes please call also.
Parking Lot Drop Off & Pick Up
In the “bus loop”, which is the first drive north on Springhurst you may load and unload in the red curb area behind the buses. You will turn left when leaving the bus loop. The main parking lot in the front of the school has a red curb, for loading and unloading only. You will turn right when leaving the main parking lot. Double parking is not permitted at any time. DO NOT leave your car unattended in any red zone at anytime. Park only in marked parking stalls. Since many of our families are located very close to Creekside, we are hopeful that children will walk to school. Parents should walk with students and cross in designated crosswalks only with an adult. A gate is located at the back of the playground to provide easy access from the park.
We have been notified by the San Diego Police Department that it is illegal to make a U-Turn at the stop sign located just south of the school at Springhurst Dr. and Bridgewood Way. For the safety of our students, and to avoid a traffic ticket, please do not make a U-Turn at this location.
Extended Student Services
Creekside provides an ESS program, which is a fee based before and after school childcare program. ESS is open from 6:30 A.M. to 6:00 P.M. All students planning to participate in ESS must be registered at our ESS building. Please call our coordinator, Donnie Kirchmeier at 858-679-9601, for information regarding registration and fees.
