NEW STUDENT ENROLLMENT for 2012 – 2013

Kindergarten News: The new student enrollment begins from May 8th. Please ensure you have the following documents ready on the day of registration:

Kindergarten Enrollment
Tuesday, May 8th 8:30 am -11:00 am

• Completed enrollment form (click here to access forms)
  
  Child's official birth certificate
  (your child must be 5 years old on or before Dec2, 2012)
  
  Child's Immunization Record
  (5 doses DPT (4 doses if last dose was given after 4th birthday).4 doses Polio (3 doses if last dose was given after 4th birthday).3 doses Hepatitis B. 2 doses MMR. Varicella (chicken pox) immunization or physician’s documentation that child has had the disease click here
  
  2 documents showing proof of residence
  (utility bills, bank statement, lease or rental agreement, escrow paper

5th Grade end-of-the-year Activities
   
    black box (magazine holder, actually) in the workroom that is labelled "5th grade promotion and picnic"....you can     put any money, pics, questions, etc in that box and we will check it often.
