Kindergarten Enrollment for the 2012-2013 school year begins May 7 through May
11, 2012 from 9:00 a.m. – 2:00 p.m. in the Office

Child's official birth certificate
(your child must be 5 years old on or before Dec 2, 2012) -
Child's Immunization Record
5 doses DPT (4 doses if last dose was given after 4th birthday).
4 doses Polio (3 doses if last dose was given after 4th birthday).
3 doses Hepatitis B.
2 doses MMR. Varicella (chicken pox) immunization or physician’s documentation that child has had the disease -
2 documents showing proof of residence
(utility bills, bank statement, lease or rental agreement, escrow paper)

IT’S OFFICIAL… MIDLAND ELEMENTARY is a NO EXCUSES UNIVERSITY school!! The No Excuses University (NEU) network of schools has accepted Midland Elementary into this cadre of elementary, middle and junior high schools across the United States who actively promote a comprehensive model of college readiness to all students the moment they enter school. Midland Elementary is the 100th school to achieve this NEU accomplishment, and the second school in the Poway Unified School District to join the network.
