OVMS New Student Enrollment

(Continuing Student Registration Information Coming Soon.
All paperwork will be collected on our Registration dates:  August 13th for 6th grade,
August 14th for 7th grade, and August 15th for 8th grade)

We will start collecting new student (students new to the Poway Unified School District) enrollment paperwork for the 2012-13 school year in May, 2012.

Once you have filled out the required Poway Unified School District forms (available here), please return to OVMS website for our paperwork (see below).

2.  Oak Valley Middle School Placement Form

3.  Oak Valley Signature Form (2011/2012)

     a.  Read Oak Valley Family Handbook (2011/2012)

     b.  Read Literacy Letter (2011/2012)

4.   Pick applicable year and grade level form below

      a. 2012-2013 6th Grade Course Request Form

      b. 2012-2013 7th Grade Course Request Form

      c. 2012-2013 8th Grade Course Request Form

      d. 2011-2012 6th Grade Course Request Form

      e. 2011-2012 7th Grade Course Request Form

      f. 2011-2012 8th Grade Course Request Form

Be sure to have your student sign the Course Request Form as well as the Oak Valley Signature Form and PUSD Signature Verification Form

After you have completed filling out the paperwork, it will be necessary for you to call the Welcome Center (office at OVMS) to make an appointment to submit your paperwork (takes about 30 minutes).
