Athletic Handbook

Please read the Athletic Handbook before you complete the forms required to receive your Ticket to Play.  You and your son/daughter are responsible for the information that is contained in the Athletic Handbook.

Ticket To Play

To get your "Ticket-to-Play", Print and fill out all forms listed below. All Ticket-To-Play forms must be updated each school year. Physicals are good for 1 year from date of examination and Can Not expire during sports season. Only PUSD Athletic Screening History & Physical Exam forms are accepted. Bring ALL signed forms to the Athletic Office to receive your ticket.
