2012-2013 Enrollment Procedures

New Student Enrollment forms will be available at the Poway website:  http://www.powayusd.com/enrollment/. Please download the 2012-2013 required forms and bring them with you when you enroll.  Kindergarten (only) enrollment will begin on May 8, 2012 from 2:30 to 5:00 PM in Multi Purpose Room. Beginning May 9, 2012 enrollment will be in the Park Village office from 9:30 to1:30. Enrollment for grades 1 to 5 will begin May 9, 2012 from 9:30 to 1:30 in the Park Village office.

Checklist of documents to bring with you to enroll your incoming kindergartener or other student new to Park Village

____ Original birth certificate or passport and a copy

____ Download and fill out all of the required 2012-2013 enrollment forms at http://www.powayusd.com/enrollment/and site specific forms found on the Park Village website.


____ Two separate forms of residency verification and a copy of each (Example: deed to primary residence, lease agreement, utility bill)

____ Original current immunization records and a copy.

____ Contact information for local relatives or friends who will be allowed to pick your child up from school

Listed below are the site-specific forms necessary to complete your student’s registration for the 2012-13 school year. CLICK on each form, complete, print and bring with you to enrollment

Logon to the Poway District website at http://www.powayusd.com/registration/ to complete AND PRINT the required District forms for CONTINUING STUDENT REGISTRATION.

KINDERGARTEN ENROLLMENT

Beginning May 8, 2012 2:30 To 5:00 PM

To enroll your child, you must bring proof that the following immunizations have been administered:

• 4 Doses - Polio (3 doses if at least one dose was given on or after fourth birthday)
• 5 Doses - DPT (4 doses if at least one dose was given on or after fourth birthday)
• 2 Doses - MMR (Both doses must be given on or after first birthday)
• 3 Doses - Hepatitis B
• 1 Dose - Varicella (or health care provider documentation of chicken pox disease or immunity) Official immunization record must include:
• Name of Child
• Date of Birth
• Name of immunization administered
• Month and year of EACH immunization. Include day if obtained within birth month.
• Name of health care provider or agency administering EACH vaccine.

All immunizations need to be up to date.
