2012-2013 Continuing Student Registration Procedure
SCHOOL REGISTRATION
CONTINUING STUDENT REGISTRATION (currently enrolled at Park Village) must REGISTER for the new school year IN ORDER TO BEGIN SCHOOL ON AUGUST 20, 2012. REGISTRATION will be held on Tuesday, May 15 A-L & May 22 M-Z between 2:30 am – 4:30 pm in the MPR. This date replaces the registration that was previously held in August. Remember you must REGISTER your student for the new school year (2012-2013) IN ORDER TO BEGIN SCHOOL ON AUGUST 20, 2012.
REGISTRATION is the process by which we update personal information such as address, email address, phone numbers, emergency contacts, etc. You will be required to Correct, Sign and Date the registration document provided. You also must bring all documents to Park Village on this day. The following are the documents you are REQUIRED to bring with you:
- Signature Verification Form (Must be signed by parents and 4th & 5th grade students ONLY.)
- Health Information Form (Required for 4th grade students ONLY however, the office is requesting a form from every student to keep your child’s health information more up-to-date.)
- Walking field trip form
All District and Park Village registration forms must be completed. Incomplete registration forms and documentation will result in a student NOT being registered at Park Village Elementary School.
**NOTE: You will be able to print out the forms from either the Park Village Website or the Poway Unified School District website. Please watch for the 2012-2013 forms to be uploaded (around May 1) before downloading the forms. Please give yourself ample time to complete these forms.

Registration for the 2012-2013 school year. Print documents for both the district (link at the bottom of this page) and Park Village site and bring them to Park Village:
A-F 2:30 to 3:30, May 15
G-L 3:30 to 4:30, May 15
M-R 2:30 to 3:30, May 22
S-Z 3:30 to 4:30, May 22
This date replaces the registration that was previously held in August. Students (currently enrolled at Park Village) must be REGISTERED for the new school year (2012-2013) IN ORDER TO BEGIN SCHOOL ON AUGUST 20, 2012. We are asking that you plan to register at the appropriate time alphabetically. This will help the process go quicker and save long lines.
This process also ensures that you are informed of school rules, state laws, your rights, and emergency procedures.
