KINDERGARTEN ENROLLMENT
Kindergarten Enrollment for the 2012-2013 school year begins Wednesday, May 9th from 9:00 a.m. – 12:00 p.m. in the Office and continues through Jun 19th. Please click the link below for all requirements and necessary forms.
WHAT TO BRING:-
Completed enrollment form (click here to access forms)

Child's official birth certificate
(your child must be 5 years old on or before Dec 2, 2012) -
Child's Immunization Record
5 doses DPT (4 doses if last dose was given after 4th birthday).
4 doses Polio (3 doses if last dose was given after 4th birthday).
3 doses Hepatitis B.
2 doses MMR. Varicella (chicken pox) immunization or physician’s documentation that child has had the disease -
2 documents showing proof of residence
(utility bills, bank statement, lease or rental agreement, escrow paper)
