2012-2013 New Student Enrollment

Welcome to Tierra Bonita! The following links will assist you in enrolling your new student. All new students must complete both the District forms on the PUSD enrollment page and our school site specific forms at the bottom of this page.

Students are considered "enrolled" when all District and site specific forms have been completed and returned to us and the following documentation has been provided:

• Birth Certificate
• Immunization Records
• Residency Verification
• Physical Exam- Child Health and Disability Prevention (CHDP), Required for students entering 1st grade
• Dental Screening- Oral Health Assessment, CA requirement for students entering a CA school for the first time
• Past School Information, if applicable

Also, please note that enrollment is different than registration, which is required of all new and returning students and takes place in August. Please remember that you must also complete the registration process in August to verify your student will be joining us for the new school year.

K-5 New Student Enrollment
Please join us on Tuesday, May 8, 2012 from 8am - noon in the Multi-Purpose Room to enroll your new student for the 2011-12 school year. Please bring your completed district and site specific forms, along with the required residency, immunization, and birth date documents. Please note that entering kindergarteners must be 5 years old by December 2, 2012.
