This is an information meeting for parents giving an overview of our school and kindergarten curriculum.  You will have a chance to meet our wonderful kindergarten team and tour our beautiful campus.

KINDERGARTEN ENROLLMENT- UPDATE

Willow Grove is currently at capacity. As was the case last year, we will not be accepting enrollment forms on a first come – first served basis.  All students will be placed on a Provisional Placement status.  For more information see 2012-13 Provisional Placement.  For Kindergarten enrollment, we will be implementing the following process:

Kindergarten Enrollment

May 10, 2012, 2:30-5:30 p.m. May 11, 2012, 6:30am-8:30am; and 11:30-2:30pm

You may complete and submit the enrollment forms any time on these dates during the times specified above. The following documents MUST accompany the Registration Form: Original and one(1) copy of Birth Certificate Original and one (1) copy of Immunization Records 2 Original Proofs of Residency plus one copy of each The school will NOT be accepting forms on a first come –first served basis. During this registration period all students will be enrolled on a provisional basis. In the event we do not have enough space to enroll all Kindergarten students residing within the Willow Grove attendance area, the District may consider implementing a lottery or redirecting students to other schools in the area. Please continue to check the Willow Grove website as we will be posting updates about Kindergarten enrollment. You may complete and submit the enrollment forms any time on these dates during the times specified above.  Willow Grove Elementary is currently at capacity in grades 1-5.  Enrollment will be on a first come-first serve basis.  All new enrollments will be on a Provisional Placement status.  You will be informed of your child's placement before school starts in August.  For more information see 2012-13 Provisional Placement.

Enrollment Forms

Please visit the Poway Unified School website at http://www.powayusd.com/Enrollment/ for the 2011-2012 enrollment forms. Forms for the 2012-13 school year are not available at this time.  They are expected to be available on the District website May 1, 2012. Having the forms completed ahead of time and all documentation up to date is required to complete enrollment and will save time.

REQUIRED DOCUMENTS TO BRING FROM HOME AT TIME OF ENROLLMENT

  • Birth certificate, Baptismal Certificate or Passport + 1 copy
  • Original Immunization Record + 1 copy
  • Two (2) separate forms of residency verification + copy of each form of residency       (Example: deed to primary residence, lease agreement, utility bill)
  • Kindergarten Consideration - Kindergarten Enrollment Only

PLAN AHEAD…Have immunizations completed.

All Children are required to have their vaccinations fully completed before enrollment in Kindergarten, unless exempted as provided by law.

  • 4 Doses - Polio (3 doses if at least one dose was given on or after fourth birthday)
  • 5 Doses - DPT (4 doses if at least one dose was given on or after fourth birthday)
  • 2 Doses - MMR (Both doses must be given on or after first birthday)
  • 3 Doses - Hepatitis B
  • 1 Dose - Varicella (Or health care provider documentation of chicken pox disease or immunity)

Official immunization record must include:

  • Name of Child
  • Date of Birth
  • Name of immunization administered
  • Month and year of EACH immunization. Include day if obtained within birth month.
  • Name of health care provider or agency administering EACH vaccine.

If you are taking your child to the doctor for their five year old physical and immunizations prior to enrollment, you may download the Report of Health Checkup for School Entrance MCH77 from the Poway Unified School District website (http://www.powayusd.com/enrollment/) under new student enrollment. The Oral Health Checkup Form is also available on the website.
