TICKET TO PLAYTo get your “TICKET-TO-PLAY”, fill out and print all 9 pages listed below. All Ticket-to-Play forms must be updated each school year. Physicals are good for 1 year from date of examination and cannot expire during the sports season. Only PUSD Athletic Screening History & Physical Exam forms are accepted. Bring ALL signed forms to the Athletic Office to receive your ticket: 1. Physical & Medical Information Release Form (7 pages) 2. Athletic Participation Signature Forms (2 pages with signatures): a. Athletic Handbook, Hazing and Ethics Signature Form (1 page) b. Residency & Eligibility Verification Form (1 page) |

Alternate Transportation Request Form
This form needs to be filled out BEFORE an athlete is allowed to take alternative transportation to or from a sporting event.
Directions for filling out this form -
1. Parent - fill out this form and sign it.
2. Student - take the form to your coach for signature.
3. Student - take the form to the Athletic Director for signature.
