2011-2012 NEW Student Enrollment
Enrollment forms are available at http://www.powayusd.com/enrollment/ . Please note the immunization requirements below; students will not be permitted to enroll until these requirements are met. In addition, you must bring with you the child's birth certificate or passport, two separate forms of residency verification, and the completed immunization records.
2012-2013 Enrollment for New Students
Forms for New Student Enrollment for the
2012-2013 school year will be available at the PUSD Enrollment link, on May 1, 2012.
Kindergarten enrollment will begin on Wednesday, May 9, from 11-4 in the office and continuing between 9:30-11:30 daily through June 15.
Enrollment for new 1st-5th grade students will begin on Monday, May 14 and ongoing through June 15
All paperwork must be completed before enrollment is accepted. Please refer to the instructions on the PUSD Website for more information. Enrollment forms will be available at www.powayusd.com beginning in April 2012.
Please bring:
Original Birth certificate, current passport or baptismal certificate
Immunization Records (K-12)- click here for current immunization

What to Bring on designated date:

Grades K-5: Completed enrollment forms. You may download the forms on the PUSD link .

Kindergarten only: Your child's official birth certificate (we cannot accept the hospital certificate). Your child must 5 years old on or before December 2, 2011 (for 2011/12 enrollment).

Grades K-5: Your child's immunization record. Requirements are as follows:
- 5 doses DPT (4 doses if last dose was given after 4th birthday)
- 4 doses Polio (3 doses if last dose was given after 4th birthday)
- 3 doses Hepatitis B
- 2 doses MMR
- Varicella (chicken pox) immunization or physician's documentation that child has had the disease.

All children are required to have their vaccinations fully completed before enrolling in Kindergarten, unless exempted as provided by law. Only two types of exemptions are allowed for immunization requirements: (1) medical, or (2) personal beliefs. Exemptions must be handled prior to enrollment. Call the school office at 858-484-0981 regarding exemption forms.

The State of California requires all 1st grade students to have a physical on file with the school (may be completed up to 18 months prior to or within 3 months of starting 1st grade) as well as an Oral Health Assessment (due on or before May 31st of their Kindergarten year). You can access both of these forms (CHDP & Oral Health Assessment) on the PUSD website. http://www.powayusd.com/enrollment
Grades K-5: Two documents showing proof of residence. This can include utility bills, bank statement, lease or rental agreement, escrow papers. We cannot accept a driver's license or car registration as proof of residence.
We cannot enroll your child unless you have all of these documents with you at the time of registration. Please bring original documents with you, plus copies that we can keep.
